Personal Assist South Yorkshire Rehabilitation Team
The Personal Assist South Yorkshire Rehabilitation Team is a specialised service within our healthcare team in South Yorkshire, aimed at providing rehabilitation services to individuals recovering from illness, surgery, or injury. The service is designed to allow people out of hospital sooner to free-up beds and give a short to mid term service to individuals that will go on to no longer requiring care or alternatively be assessed for longer term care.
This team consists specialist trained care workers who form a part of a multidisciplinary team of healthcare professionals such as physiotherapists, occupational therapists, nurses, and sometimes speech therapists, who work together to help service users regain independence and functionality.

Personal Assist South Yorkshire helps service users with rehabilitation through a person-centred approach, designed to promote independence, improve quality of life, and support physical and emotional recovery. Here's how they might assist their service users in rehabilitation:
Personalised Care Plans
Assessment of Needs: Personal Assist South Yorkshire would assess each individual’s unique needs, taking into account their medical condition, level of mobility, mental health, and daily living abilities.
Tailored Rehabilitation Programs: Based on this assessment, they create a personalised rehabilitation plan, which could include physical exercises, occupational therapy, and other types of support specific to the user’s condition.
Physical Rehabilitation
Physiotherapy: To help service users regain strength, mobility, and balance after surgery, injury, or illness. This could involve tailored exercises and guided physical therapy sessions aimed at improving movement and preventing further injury.
Exercise Programs: These might be home-based exercises or programs done in a controlled environment, aimed at improving fitness levels, flexibility, and strength.
Occupational Therapy
Daily Living Skills: They assist users in relearning or improving skills needed for daily living (e.g., dressing, cooking, grooming). This is particularly important for individuals who’ve had strokes, accidents, or surgeries that impair their ability to perform these tasks.
Home Adaptations: Occupational therapists might assess the user’s home environment and recommend modifications (e.g., grab rails, ramps, accessible showers) to ensure it is safe and conducive to recovery.
Emotional and Mental Health Support
Psychological Support: Rehabilitation can be mentally challenging, and Personal Assist South Yorkshire may provide access to counsellors or therapists who help users cope with the emotional impact of their health condition.
Motivation and Confidence Building: They may work to boost users' confidence by setting achievable goals and celebrating progress, ensuring users stay motivated throughout their rehabilitation journey.
Community Support and Integration
Social Support: Helping service users re-integrate into their communities, whether through social activities, support groups, or helping them build new routines. They may connect users to local community services or clubs to aid in social recovery.
Access to Local Resources: The organization might assist in securing resources like mobility aids (wheelchairs, walkers), transport services, or meals to aid daily living during rehabilitation.
Ongoing Monitoring and Adjustments
Regular Reviews: They likely provide regular follow-ups to monitor progress and adjust care plans as needed, ensuring that rehabilitation continues to be effective as the user’s needs change.
Feedback Mechanisms: By listening to service users and adjusting support based on their feedback, the team can provide better-personalized care, ensuring they achieve their rehabilitation goals.
Carer and Family Involvement
Family Training and Support: If family members or informal carers are involved, Personal Assist may offer training to ensure they know how to support the user in their rehabilitation process. They might also offer emotional support to family members.
Assistance with Mobility Aids and Technology
Adaptive Equipment: Personal Assist South Yorkshire might help service users gain access to mobility aids (e.g., walking sticks, wheelchairs) or assistive technology (e.g., devices to help with speech or memory).
Assistive Technologies: The use of smart devices or alarms that help people with limited mobility, cognitive impairments, or other issues live more independently.
End-of-Rehabilitation Support
Transition to Independent Living: When users complete their rehabilitation, the team might work on plans for transitioning back to full independence, or ensure ongoing support is available if required.
This comprehensive, holistic approach focuses on all aspects of a person’s life, making sure they receive the physical, emotional, and social support necessary for a successful rehabilitation.
